SUBJECT: GUIDELINES ON THE RELEASE OF CHECKS FOR SALARIES AND ALLOWANCES [MODIFIED DISBURSEMENT SCHEME (MDS) AND CONTINUOUS FORMS] OF JUDGES AND PERSONNEL OF THE LOWER COURTS. Checks for salaries and allowances of judges and personnel of the lower courts, shall be released through mail. However, such checks may be personally taken from the Checks Disbursement Division of the Office of the Court Administrator subject to the.following conditions: The employee concerned may have his/her check released through his/her legitimate spouse, son or daughter duly authorized under a Special Power of Attorney. Except for those herein mentioned, no other person may be authorized by the employee to take his/her check. The authorized representative shall present a copy of the Special Power of Attorney and an identification card when taking the check of the principal. Checks for salaries and allowances shall be released three [3] days before due date. This circular shall take effect on April 1, 1997. March 31, 1997.

As such, he/she is given authority by the undersigned to get this court’s checks representing salaries, allowances, and other claims from 19__ to _________19__.
